To expedite your appointment with our oral surgeon, we have made some forms available to download and print for you to fill-out and to bring on the date and time of your appointment. We invite potential clients to explore our website to learn more about our procedures which may help you to better prepare for your visit.
If applicable, please provide: Doctor’s referral including any x-rays, a list of medications you are currently taking and a list of drug allergies and have this information with you when you come in for your appointment.
*A parent or guardian must be present at the consultation visit and surgery appointments for ALL patients under the age of 18.
Your initial appointment will consist of a consultation with Dr Simon Y Bangiyev. During this time, he will explain your diagnosis in addition to the treatment options available. Depending on the circumstances and availability, the procedure may be able to be scheduled on the same day as the evaluation.
Serving patients throughout Norwalk, Stamford, and Westport, CT, our office is equipped with complete digital radio graphic capability. We encourage patients and provider offices to forward any images to us electronically or by mail. Any document sent should include: the patient’s first name, last name and date of birth.

Payment Information
Full payment is due at the time of service unless prior arrangements have been made. If you have insurance, deductible and co-payments are due at the time of service.
The patient is responsible for all fees incurred in the office. Any service not covered by insurance will be at the responsibility of the patient. At Premier Oral Surgery, we understand that the financial aspects of healthcare can be difficult to understand and confusing. Our friendly office staff will try to do everything we can to help you.
